Ward to stipulate on request form if "pre-dose" or "post-dose".
Random levels- take as requested.

Pre-dose (trough level) - immediately before next dose is given.
Post-dose (peak level) - 30 minutes after the end of an IV dose or 60 minutes after an IM dose.

Route of administration, dose given and time of last dose must be noted on request slip.

GENERAL NOTES



ONCE DAILY DOSING:
Single serum level collected 6-14 hours post-dose is compared to the graph on page 232 of the Australian "Therapeutic Guidelines Antibiotic" and dose adjusted accordingly.

8-12 HOURLY DOSING:
Recommended trough < 2 mg/L
Recommended  peak > 8 mg/L
